 The Maturational Interpretation

1.       History

1.1.    Half a century ago it was thought that what healed a patient was the recall of memories.

1.2.    When it became evident that the memories were less important than what prevented their recall, interpretations were made to overcome the repressive forces in the guise of resistance.

1.3.    Later resistance was recognized as an essential source of interpretive data because it told the story of the ego’s development.  The focus shifted to the constrictive influence of the resistance. Explanations were oriented toward the integration of the ego and the acquisition of insight.

1.4.     More recently, widening appreciation of the communication function of resistance has stimulated other approached to interpretation.

1.5.    The use of interpretation primarily for veil-lifting purposes is waning, with the recognition that other aspects of treatment relationship are often more significant than the self-understanding.

1.6.    The patient usually acquires self-understanding, but it is rarely the decisive factor in the case.

 

2.       INADQUACY OF OBJECTIVE UNDERSTANDING

2.1.    Instead of trying to overcome resistance by explaining problems, the analyst uses interpretation to create the precise emotional experience that will resolve the problem.

2.2.    Spotnitz found it helpful to operate on the hypothesis that interpretation is consistently employed for maturational purposes.

2.3.    Deleterious experiences with its natural objects caused fixations or arrests in growth.

2.4.    In attempting to coping with them, he developed maladaptations: certain repetitive patterns that drained off energy needed for maturation into circuitous processes.

 

3.       REVERSAL OF MALADAPTATIONS

3.1.    Maladaptations are not totally reversible, but it is sufficient for the analyst to intervene to loosen the compulsive grip and nullify the effects of the original blockages.

3.2.    If he does so, the patient usually requires little help in obtaining and assimilating experiences that will reduce his maturational needs.

3.3.    The therapist intervenes to lay the foundation for new growth by freeing the patient from the stranglehold of the pathological maladaptations.

3.4.    By and large, maladaptations patterns are dealt with when they have been reactivated with sufficient intensity to be reducible. (부부 싸움)

3.5.    Preverbal patterns are responsive only to symbolic, emotional, and reflective interventions.

3.6.    Affective nonverbal communications, even the analyst’s state of being, give the preoedipal personality freedom to grow.

3.7.    During this preparatory period, the emotional logic of the patient’s behavior on the couch is not explained to him.

3.8.    After the preoedipal maladaptations are more or less resolved, the patient becomes accessible to verbal interpretation.

 

4.       MOTIVATION AND TIMING

4.1.    Interpretation is ineffective unless it is motivated by a specific therapeutic intent.

4.2.    The response of the patient is a more important consideration that the accuracy of the explanation.

4.3.    The analyst should be able to anticipate the patient’s reaction to an interpretation, and then voice it only if the anticipated reaction would be desirable at that time.

4.4.    Intuition, inspiration, and empathy are among the personal qualities required for the art of interpretation.
